From what I understand it's real time, like the main mode in FoT. Same thing: Agi gives you action points, and it regenerates said action points over time. The whole V.A.T.S thing (Bullet-time sort'a thing) just makes it so you don't have to twitch-game to play. It does the old aimed shot mechanic for you, in real time, automatically, at the cost of AP. That's my understanding of it, so far.
